The Coal Mining History Resource Centre has a very good site http://www.cmhrc.co.uk/site/home/ which includes a national database of mining deaths in the UK. It's well worth a look.
As you know, I have a (Maddox) interest with this Thomas Thomas. I've already searched but haven't found any connection with Thomas' death. It appears he died in Caerphilly but was buried in the Forest of Dean.
